Phentermine, Topiramate falls under CNS DEPRESSANTS — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might have additive effects if used with other CNS depressants.
Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

Here's the thing with combining CBD and Qsymia (phentermine/topiramate): the concern is really about the topiramate side of this pill. Topiramate can already make people feel foggy, drowsy, dizzy, or just mentally slowed down. CBD, especially at higher concentrated doses, can have its own calming, sleepy effect. Put them together and those effects may stack up, so you could feel more tired, woozy, or hazy than usual.

I'll be honest, this is mostly a theoretical caution based on how these substances work, not strong proof from big studies. The phentermine part is a stimulant, so things can be a bit unpredictable. Go easy, watch how you feel, and check with your pharmacist or doctor before pairing them.

Theoretically, CYP2C19 inhibitors might increase cannabidiol levels.
Cannabidiol is a substrate of CYP2C19 enzymes. Theoretically, drugs that inhibit CYP2C19 enzymes might increase levels of cannabidiol, increasing its effects and adverse effects.

Here's the key thing to know about pairing CBD with Qsymia (phentermine/topiramate): it's actually the CBD that may be affected, not your weight-loss medicine. The topiramate part of Qsymia can mildly block an enzyme called CYP2C19, which is one of the pathways your body uses to clear CBD. So in theory, CBD could build up a bit and its effects (and side effects) could feel stronger than expected.

What might that look like? Things like extra drowsiness, tiredness, dizziness, or stomach upset. I'll be honest with you though, this is only a theoretical concern based on how the enzymes work, not from real-world reports, so it may not amount to much. Still, since both can cause sleepiness and dizziness, it's worth a quick chat with your pharmacist or doctor before combining them.

Cannabidiol might increase topiramate levels.
In clinical research, concomitant administration of cannabidiol and topiramate, a CYP2C9 and CYP2C19 substrate, is associated with a modest increase in plasma levels of topiramate. Topiramate levels stayed within the normal range and did not require dose adjustment. However, caution should be exercised when cannabidiol and topiramate are taken together.

Qsymia is a combo of phentermine and topiramate, and it's the topiramate piece that matters here. Your body clears topiramate partly through liver enzymes (called CYP2C9 and CYP2C19), and CBD can slow them down a bit. The result is that topiramate may build up slightly, so its effects could feel a little stronger.

The good news is that in actual clinical research, this bump was modest and topiramate stayed within its normal range, so no dose change was needed. Still, it's worth watching for topiramate-type effects like tingling in your hands or feet, trouble concentrating, drowsiness, or changes in taste. If you notice these creeping up after adding CBD, check in with your pharmacist or doctor before changing anything.
