Ponesimod falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the basic idea: your liver uses an enzyme called CYP3A4 to help break down Ponesimod (Ponvory). CBD can slow that enzyme down, so the medicine may not clear as quickly and its levels in your blood can creep higher than expected. Ponesimod is an active drug, not a prodrug, so more of it in your system means a stronger effect and a bigger chance of side effects.

For Ponesimod, that could mean things like a slow heart rate (especially when first starting it), shortness of breath, dizziness, headache, or higher blood pressure. The evidence here is solid for CYP3A4 in general, though it hasn't been tested directly with Ponesimod. Please don't change anything on your own. Check with your pharmacist or doctor first.

Ponvory (ponesimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes clinically isolated syndrome (a first MS episode), relapsing-remitting MS, and active secondary progressive MS.

It is not approved for children, and it is only available as an oral tablet under the brand name Ponvory.
