Here is the interaction we found between these two, with the evidence behind it.

STIRIPENTOL (Diacomit)

Cannabidiol can increase stiripentol levels.
Two clinical pharmacokinetic studies in patients stabilized on stiripentol shows that adding cannabidiol, 750 mg twice daily for 3-10 days or up to 20 mg/kg daily for 24 days, increases the average maximum concentration of stiripentol by 17% to 28% and the average area under the curve by 30% to 55% when compared with taking stiripentol alone. The mechanism for this interaction is unknown; cannabidiol might inhibit cytochrome P450 2C19 (CYP2C19) and/or UDP-glucuronosyltransferase (UGT) isoforms, which metabolize stiripentol. Although there were no adverse clinical outcomes, caution should be exercised when cannabidiol and stiripentol are taken together.

Here's the short version: if you add CBD to your stiripentol, the CBD can slow down how your body clears the stiripentol. In real clinical studies, blood levels of stiripentol rose by roughly 17% to 55%. That's because CBD seems to block some of the enzymes (CYP2C19 and certain UGTs) that normally break stiripentol down, so more of it sticks around.

More stiripentol in your system can mean stronger side effects like drowsiness, dizziness, poor balance, reduced appetite, or upset stomach. Nobody in the studies ran into a serious problem, but this is a real, well-documented interaction. Since stiripentol is part of a carefully balanced seizure plan (often with clobazam), please don't add CBD on your own. Talk with your doctor or pharmacist first so they can watch your dose.

What Is Stiripentol Used For?

Brand name: Diacomit

Diacomit (stiripentol) is used to treat seizures linked to Dravet syndrome — a rare, severe form of epilepsy that usually begins in infancy. It is approved for patients 6 months of age and older who weigh at least 7 kg and are already taking the seizure medicine clobazam.

Diacomit is not approved for use on its own (monotherapy) in Dravet syndrome. It must always be used alongside clobazam as part of a combination treatment plan.
