Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist: your body relies on an enzyme called CYP3A4 to clear cariprazine (Vraylar) out of your system. Lab studies suggest that compounds in cannabis can slow that enzyme down a bit. If that happens, cariprazine could linger longer and build up, which may nudge up its usual effects and side effects, things like drowsiness, restlessness, dizziness, tremor, or trouble with movement.

I want to be honest about the strength of this, though. It's based only on test-tube research, not real-world cases, so it stays in the theoretical zone. Cariprazine also hangs around the body a long time on its own, so any change may be hard to notice. Still, if you use cannabis, mention it to your doctor or pharmacist so they can keep an eye on how you're feeling.

Here's the good news on combining cannabis with your Vraylar (cariprazine): the research we have doesn't show cannabis changing the blood levels or the effectiveness of antipsychotics. Studies looked at similar drugs like aripiprazole and olanzapine and found no meaningful effect, so cannabis is unlikely to make your cariprazine weaker or stronger from a chemical standpoint.

That said, this evidence is fairly limited, and there's a more practical concern. Both cannabis and cariprazine can affect your thinking, mood, and alertness. Cannabis (especially THC) can sometimes worsen anxiety, paranoia, or mood swings, which may work against what your medication is trying to do. If you use or are thinking about cannabis, please loop in your doctor or pharmacist so you can keep an eye on how you're feeling.

What Is Cariprazine Used For?

Brand name: Vraylar

Vraylar (cariprazine) is approved to treat schizophrenia in adults and teens 13 and older, and to treat acute manic or mixed episodes associated with bipolar I disorder in adults and children 10 and older. In adults, it is also approved for depressive episodes associated with bipolar I disorder (bipolar depression).

Additionally, Vraylar is approved as an add-on therapy alongside antidepressants for adults with major depressive disorder (MDD) who need extra help beyond their current antidepressant alone.
