Delta-9-tetrahydrocannabinol (THC), an active constituent of cannabis, is a substrate of CYP3A4 enzymes.

Here's the gist for this combo: erythromycin is one of those antibiotics that slows down a liver enzyme called CYP3A4. Your body also uses that same enzyme to clear THC, the part of cannabis that gets you high. So when you take erythromycin alongside cannabis, the THC may hang around a bit longer and feel stronger than usual. Think more pronounced effects like drowsiness, dizziness, a faster heartbeat, dry mouth, or feeling more impaired than you expected.
I'll be honest with you, this one is mostly theoretical and based on how the drugs work on paper, not on strong human studies, so it may not amount to much for everyone. Still, if you use cannabis regularly, it's worth a quick chat with your pharmacist or doctor, and go easy until you know how you react.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

P-GLYCOPROTEIN SUBSTRATES

Most in vitro research suggests that constituents of cannabis, including cannabidiol (CBD) and delta-9-tetrahydrocannabinol (THC), can inhibit P-gp and increase the accumulation of probe compounds by reducing P-gp mediated drug efflux. In vitro studies in kidney cell lines show that a 1-hour exposure to CBD and THC inhibits P-gp. Cannabis may also alter the expression of P-gp, although this effect appears to vary based on duration of exposure. Some in vitro research in lymphoblastoid leukemia cell lines indicates that a 1-hour exposure to cannabinoids does not affect P-gp expression, while a prolonged 72-hour exposure decreases P-gp expression. Other in vitro research in these cell lines shows that a 4-hour exposure to THC and CBD induces P-gp gene expression, while exposure for longer than 4 hours and up to 48 hours does not induce P-gp gene expression.

Here's the gist with cannabis and your erythromycin (E.E.S.). Erythromycin is carried around your body partly by a little pump called P-glycoprotein (P-gp), which helps move the drug out of cells. Lab studies suggest that the THC and CBD in cannabis may slow that pump down. In theory, that could let a bit more erythromycin build up in your system, which might mean more of its usual side effects, mainly stomach upset, nausea, cramping, or diarrhea.
I want to be honest about the strength of this, though. This is only based on test-tube and cell studies, not real people, so we don't truly know if it matters in everyday use. It's worth mentioning to your pharmacist or doctor, especially if your stomach feels rough, but don't change anything on your own.
