Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the thing about bosentan: it can already be hard on the liver, which is exactly why your doctor checks your liver enzymes with regular blood tests while you're on it. Comfrey is the worry here because the compounds it contains (called pyrrolizidine alkaloids) can also injure the liver, especially when it's swallowed. Stacking two liver stressors together could, in theory, add up and raise your risk of liver trouble.

To be fair, this concern is theoretical and based on how each one behaves, not on proven cases of the two combined. Still, comfrey should never be taken by mouth, and even on the skin it deserves caution. Please loop in your pharmacist or doctor before using any comfrey product while you're taking bosentan.

Comfrey is a traditional herb used mainly on the skin for bruises, sprains, and joint pain, and some topical products show modest benefit for these uses. However, comfrey contains compounds that can seriously damage the liver, so it should never be taken by mouth and should only be used on unbroken skin for short periods. Pregnant or breastfeeding people should avoid it entirely.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
