Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the thing to keep in mind if you take bosentan (Tracleer) for pulmonary arterial hypertension. Adrafinil is converted by your liver into modafinil, and modafinil can nudge your blood pressure up a bit, both the top and bottom numbers. Bosentan works in the opposite direction by relaxing the blood vessels in your lungs, so in theory adrafinil could work against the pressure-lowering goals your doctor is aiming for.

I'll be honest, this concern is theoretical and based on how these drugs behave, not on strong studies in people taking them together. The effect, if any, is likely small. Still, since adrafinil is unregulated and hard on the liver, please loop in your doctor or pharmacist before mixing it with bosentan.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the gist for Bosentan (Tracleer). Your liver uses an enzyme called CYP2C9 to help clear bosentan from your body. Adrafinil turns into modafinil, and lab studies hint that modafinil may dial down this same enzyme. If that happens, bosentan could be cleared more slowly and build up a bit, which might make its side effects more noticeable, things like headache, flushing, swelling in the legs or ankles, low blood pressure, or feeling unusually tired.

I want to be honest with you though: this is a theoretical concern based on test-tube data, not real patient cases, so it may not amount to much. Still, since bosentan already needs liver monitoring, please run this combination by your doctor or pharmacist before mixing the two.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.

Here's the gist for Bosentan (Tracleer). Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up a liver enzyme called CYP3A4. Since Bosentan is partly broken down by that same enzyme, the worry is that your body might clear it a little faster, which could leave a bit less of it working to keep your lung blood vessels relaxed.

In plain terms, your PAH might not be controlled quite as well, meaning more shortness of breath or less stamina with activity. That said, this is a theoretical concern based on how the drugs behave, and modafinil's effect on the enzyme is weak, so it may not amount to much in real life. Still, because PAH is serious, please run this by your doctor or pharmacist before combining them.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
