Why this applies to Bosentan

Bosentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Your body relies on a liver enzyme called CYP3A4 to help clear bosentan from your system. In test-tube studies, certain compounds in cubeb berries appear to slow that same enzyme down. If that were to happen in people, bosentan could build up a bit more than expected, which might bring on more of its usual side effects like headache, flushing, swelling in the legs or ankles, or a drop in blood pressure that leaves you lightheaded.

Here is the important part: this is only laboratory evidence, and it has not been seen in humans, so I would not lose sleep over a pinch of cubeb used as a spice. Concentrated extracts or oils are a different story. Since bosentan also needs liver monitoring, it is worth a quick chat with your pharmacist or doctor before adding any cubeb supplement.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
