Dorzolamide, Timolol falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Dendrobium interacts with.

The dendrobium constituent dendrobine is reported to have convulsant effects. Theoretically, combining dendrobium with pro-convulsant drugs might increase the risk of seizure. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the gist with Cosopt (dorzolamide and timolol). The concern on file is that a compound in Dendrobium called dendrobine may, in theory, make seizures a little more likely, and some medicines are thought to lower the seizure threshold too. So the worry is about a possible additive effect, not about Dendrobium changing how Cosopt works in your eyes.

The good news is this is a theoretical flag based on pharmacology, not real-world reports, and Cosopt is eye drops, so very little reaches the rest of your body. That makes any practical risk quite low for most people. Still, if you have a seizure history or take other medicines, it's worth a quick chat with your pharmacist or doctor before adding Dendrobium.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
