Why this applies to Acetaminophen, Butalbital

Acetaminophen, Butalbital falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2E1 substrates.
Chlorzoxazone is metabolized by CYP2E1 enzymes. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of chlorzoxazone (Paraflex 250) 250 mg increases blood levels of chlorzoxazone by 34% and decreases chlorzoxazone clearance by 40%. It is speculated that diosmin reduces the metabolism of chlorzoxazone by inhibiting CYP2E1.

Here's the thing to know about this combo. The acetaminophen part of your medication is partly handled by a liver enzyme called CYP2E1. Diosmin may gently slow that enzyme down, at least in theory. Now, here's a twist worth understanding: CYP2E1 is actually the pathway that turns a small amount of acetaminophen into a harmful byproduct that can stress the liver. So slowing it down isn't necessarily a bad thing, and it shouldn't meaningfully change how well your medicine relieves a headache.

This is based on lab reasoning and one small study with a different drug, not on real reports of harm with acetaminophen. The bigger, well-known acetaminophen rule still matters most: watch your total daily dose and limit alcohol to protect your liver. Ask your pharmacist if you have questions.
