Caffeine (prescription drug) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the gist: diosmin may gently slow down a liver enzyme called CYP3A4, which is one of the pathways your body uses to clear certain medications. If caffeine relies partly on that pathway, the idea is that it could hang around a bit longer and feel stronger than usual.

In real life, that might show up as more of caffeine's typical effects: feeling jittery or wired, a racing or pounding heart, trouble sleeping, or an upset stomach. I want to be honest with you though, this is mostly theoretical. The clearest data involves a different drug (carbamazepine), and caffeine is actually broken down mainly by a separate enzyme (CYP1A2), so the real-world impact may be small. Don't stop or change anything on your own. Just give your pharmacist or doctor a heads-up so they can keep an eye on how you feel.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Caffeine Used For?

Brand names: Dextrophin, Caffeine

Caffeine citrate injection and caffeine citrate oral solution are indicated for the short-term treatment of apnea of prematurity (breathing pauses in premature babies) in infants between 28 and 33 weeks gestational age. These are prescription, hospital-administered products.

Over-the-counter caffeine tablets (such as Diurex) are used by adults to temporarily reduce fatigue and drowsiness. The Earthmed Uplift Chewable products are chewable caffeine gels for adults 18 and older. Children under 12 should not use caffeine products as a substitute for sleep.
