Enfortumab Vedotin-Ejfv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the idea behind this one. Diosmin may slow down a liver enzyme called CYP3A4, and the active cancer-killing piece of Padcev (a chemo agent called MMAE) is partly cleared by that same enzyme. So in theory, pairing them could let a bit more of that toxic payload hang around, which might nudge up Padcev's side effects like numbness or tingling in the hands and feet, skin rash, high blood sugar, tiredness, or eye problems.

I want to be honest though: this is mostly theoretical and based on lab work with a different drug (carbamazepine), so we don't really know how much it matters with Padcev. Since this is a serious cancer infusion, please run any diosmin product by your oncology team before starting it.

What Is Enfortumab vedotin-ejfv Injection Used For?

Brand name: Padcev

  • Enfortumab vedotin-ejfv injection is used to treat urothelial cancer (cancer of the lining of the bladder and other parts of the urinary tract) that has spread to nearby tissues or other parts of the body or cannot be removed by surgery, and has worsened after treatment with other chemotherapy medications or if these chemotherapy medications cannot be used for treatment. Enfortumab vedotin-ejfv injection is in a class of medications called monoclonal antibodies. It works by helping your immune system to slow or stop the growth of cancer cells.
