Why this applies to Eravacycline

Eravacycline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP3A4 substrates.
Laboratory research is conflicting with respect to the effects of diosmin on CYP3A4. Some research suggests that diosmin does not affect CYP3A4 activity. However, other research suggests that diosmin alters the metabolism of carbamazepine, a CYP3A4 substrate. Laboratory and animal research show that oral administration of diosmin for 7 days prior to oral administration of carbamazepine increases plasma concentrations of carbamazepine, decreases the clearance of carbamazepine, and decreases the formation of carbamazepine's active metabolite. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of carbamazepine 200 mg increases blood levels of carbamazepine by approximately 58% and decreases carbamazepine clearance by 42%. It is speculated that diosmin reduces the metabolism of carbamazepine by inhibiting CYP3A4. Diosmetin, a metabolite of diosmin, may also inhibit CYP3A4.

Here's the good news on this pairing: while eravacycline (Xerava) is partly broken down by an enzyme called CYP3A4, and diosmin might slow that enzyme a bit, this concern is mostly theoretical for this antibiotic. The lab data on diosmin and CYP3A4 is actually conflicting, with some studies showing no real effect at all.

Even if diosmin nudged eravacycline levels up slightly, this is a short-course IV antibiotic given by your care team in a hospital or clinic setting, not something you dose yourself at home. They monitor you while you're on it. If diosmin did raise levels, you might see a touch more nausea or stomach upset, which are this drug's usual mild side effects. Still, mention any supplements you take to your pharmacist or doctor so they have the full picture.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. What Is Eravacycline Injection Used For?

Brand name: Xerava

Xerava (eravacycline) is approved to treat complicated intra-abdominal infections (serious infections inside the belly — such as those involving the intestines, appendix, or other abdominal organs) in adults 18 and older. It works against a specific range of bacteria that commonly cause these infections.

Xerava is not approved to treat complicated urinary tract infections (cUTI). To help prevent antibiotic resistance, it should only be used when there is a proven or strongly suspected bacterial infection that it can fight.
