Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Dusty Miller interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in dusty miller.
Dusty miller contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the twist with this pairing: the concern isn't really about your cancer drug being affected. It's the other way around. Lorlatinib speeds up an enzyme called CYP3A4, and that's the very enzyme that turns the pyrrolizidine alkaloids in Dusty Miller into harmful, liver-toxic byproducts. So in theory, taking them together could ramp up the toxic breakdown products from the plant and raise the risk of liver damage.

I want to be honest about the evidence here: this is theoretical, based on lab work, not human studies. But Dusty Miller already isn't considered safe to take internally because of those alkaloids, and lorlatinib is a serious medication for advanced lung cancer. Please loop in your oncology team or pharmacist before using any Dusty Miller product. Don't change your lorlatinib on your own.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
