Dusty miller contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the gist with Dusty Miller and telotristat (Xermelo). The worry isn't that your medication becomes weaker or stronger. It's the other way around. Telotristat can act as a mild speed-up signal (an inducer) for a liver enzyme called CYP3A4. Dusty Miller contains pyrrolizidine alkaloids, and that same enzyme can turn those plant compounds into more toxic byproducts. So in theory, taking them together could nudge up the risk of liver harm from the plant itself.

This is a theoretical concern based only on lab data, not real patient reports, so I don't want to alarm you. Honestly, Dusty Miller isn't considered safe to take on its own anyway. Please check with your pharmacist or oncology team before using any product made from it.

What Is Telotristat Used For?

Brand name: Xermelo

Xermelo (telotristat) tablets are approved to treat carcinoid syndrome diarrhea in adults. Carcinoid syndrome is a set of symptoms caused by certain tumors — called neuroendocrine tumors — that release excess serotonin into the body, triggering frequent, often severe diarrhea.

Xermelo is used in combination with somatostatin analog (SSA) therapy — a class of injected medications commonly used to control carcinoid syndrome — in patients whose diarrhea is still not adequately controlled by SSA therapy alone.
