Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Here's the gist for bendamustine (Treanda, Belrapzo, Vivimusta). This chemo drug is partly handled by a liver enzyme called CYP1A2. Fo-ti, in lab and animal studies only, has shown mixed effects on that enzyme: some forms (like the raw root or water-based extracts) may slow it down, while an alcohol-based extract sped it up. Nothing like this has actually been seen in people, so this is theoretical.

If fo-ti slowed that enzyme, bendamustine could build up and worsen side effects such as low blood counts, nausea, fatigue, and infection risk. If it sped things up, the drug might work less well. Because bendamustine is a serious cancer treatment, please don't add fo-ti without first clearing it with your oncology team or pharmacist.

What Is Bendamustine Injection Used For?

Brand names: Treanda, Belrapzo, Vivimusta

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
