Why this applies to Clarithromycin
Clarithromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: clarithromycin is cleared from your body partly by a liver enzyme called CYP3A4. In lab dish studies, German chamomile appears to slow that enzyme down a bit. If that happened in real life, clarithromycin could hang around a little longer and build up, which might mean more of its usual side effects like nausea, stomach upset, an odd taste in your mouth, or, in rarer cases, effects on heart rhythm.
That said, this is only a theoretical concern. It's based on test-tube work and hasn't actually been reported in people, so a cup or two of chamomile tea is very unlikely to cause trouble. Concentrated chamomile supplements are a bigger unknown. To be safe, just mention what you're taking to your pharmacist or doctor.

German chamomile is a widely used herbal remedy taken mainly as a tea for calming, sleep, and digestive complaints. Early research suggests possible benefits for mild anxiety and some skin or digestive issues, but the evidence is limited and most uses are based on tradition rather than strong proof. It is generally well tolerated, but people allergic to ragweed or related plants and those who are pregnant should be cautious.

What Is Clarithromycin Used For?
Brand names: Biaxin, Biaxin XL
Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.
Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
