This combo pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Both already tend to make people drowsy and a little foggy on their own. The concern here is simple: goldenseal contains berberine, and in animal studies berberine had a calming, sedative effect. So in theory, stacking it on top of this medicine could add to the sleepiness.

Be honest, this is theoretical. That added sedation has not actually been seen in people, so I wouldn't lose sleep over it. Still, if you take goldenseal and notice extra drowsiness, dizziness, or trouble concentrating (especially when driving), that's worth mentioning. Please check with your pharmacist or doctor before adding goldenseal rather than changing anything on your own.

This combination pill, Triavil, pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Both rely heavily on a liver enzyme called CYP2D6 to be cleared from your body. Goldenseal can slow that enzyme down, so the medication may not be broken down as quickly and can build up in your blood.

In real life that could mean stronger versions of the usual side effects: more drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat. Since these are not gentle drugs, even a modest rise in levels can matter. The evidence here is fair, not just theory, so I would take it seriously. Please don't change anything on your own. Check with your pharmacist or doctor first.

In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist with this combo product (amitriptyline plus perphenazine) and goldenseal. Goldenseal contains alkaloids like berberine and hydrastine that, in test-tube studies, can mildly slow down a liver enzyme called CYP2C9. If that enzyme works slower, drugs it handles could build up a bit in your blood, which can mean stronger effects and more side effects.

For these two medicines, that might look like extra drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat. That said, this is only a theoretical concern. It has only been seen in lab studies, not in real people, so I wouldn't lose sleep over it. Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.
