Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: amlodipine already lowers your blood pressure by relaxing your blood vessels. Goldenseal contains a compound called berberine, and the concern is that it may have its own mild blood-pressure-lowering effect. Stack the two together and, in theory, your pressure could drop a little more than expected.

I want to be straight with you, though. This one is mostly theoretical, based on pharmacology and some berberine studies rather than solid proof with goldenseal itself. Still, if you notice things like dizziness, lightheadedness when you stand up, headache, or feeling unusually tired, those are worth paying attention to. Don't start or stop anything on your own. Just give your pharmacist or doctor a heads-up before adding goldenseal so they can keep an eye on your numbers.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist: your body uses an enzyme called CYP3A4 to break down amlodipine and clear it out. Goldenseal seems to slow that enzyme down, so the amlodipine can hang around longer and build up a bit more than usual. Amlodipine is the active drug itself (not a prodrug), so more of it in your system means a stronger blood-pressure-lowering effect.

In real life that could show up as dizziness, lightheadedness, flushing, headache, or more swelling in your ankles and feet, especially when you stand up. The evidence here is moderate, not alarming, but I'd still keep an eye on it. Please don't stop either one on your own. Just check with your pharmacist or doctor so they can watch your blood pressure and make sure you feel okay.

What Is Amlodipine Used For?

Brand name: Norliqva

Amlodipine treats high blood pressure (hypertension) in adults and children aged 6–17 years. Lowering blood pressure reduces the risk of strokes and heart attacks. Sdamlo (oral solution) is specifically indicated for hypertension in adults and pediatric patients 6 years of age and older.

Amlodipine also treats coronary artery disease in adults, including chronic stable angina (chest pain with exertion), vasospastic angina (Prinzmetal's or variant angina, caused by coronary artery spasms), and documented coronary artery disease in patients without heart failure or significantly reduced heart pumping function.
