Amlodipine Benzoate falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: amlodipine (Katerzia) already relaxes your blood vessels to bring your blood pressure down. Goldenseal contains berberine, and in some studies berberine can nudge blood pressure lower too. So the theory is that pairing the two might push your numbers down a little more than the amlodipine alone would. Amlodipine isn't a prodrug, so this is about adding effects, not blocking how your body activates the medicine.

To be fair, this is mostly theoretical and based on berberine, not goldenseal itself. Still, if your pressure drops too far you might feel dizzy, lightheaded, tired, or get a headache, especially when standing up. Please don't change anything on your own. Just check with your pharmacist or doctor before adding goldenseal.

Amlodipine Benzoate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist: your body relies on an enzyme called CYP3A4 to break down amlodipine. Goldenseal (mainly its berberine and hydrastine) tends to slow that enzyme down. When the cleanup crew works slower, more amlodipine can hang around in your bloodstream than usual.

Since amlodipine is an active drug (not a prodrug), higher levels mean its blood-pressure-lowering effect could get stronger. That can show up as dizziness, lightheadedness, flushing, headache, swelling in the ankles, or feeling unusually tired. The evidence here is moderate and this is rated as a possible interaction, so it won't happen to everyone. Still, before pairing goldenseal supplements with your amlodipine, please check with your pharmacist or doctor so they can keep an eye on your blood pressure.

What Is Amlodipine Used For?

Brand name: Katerzia

Amlodipine treats high blood pressure (hypertension) in adults and children aged 6–17 years. Lowering blood pressure reduces the risk of strokes and heart attacks. Sdamlo (oral solution) is specifically indicated for hypertension in adults and pediatric patients 6 years of age and older.

Amlodipine also treats coronary artery disease in adults, including chronic stable angina (chest pain with exertion), vasospastic angina (Prinzmetal's or variant angina, caused by coronary artery spasms), and documented coronary artery disease in patients without heart failure or significantly reduced heart pumping function.
