Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

This combination product includes codeine, which already causes drowsiness in many people. The concern here is simple: goldenseal contains berberine, and in animal studies berberine showed mild sedative effects. If you stack that on top of codeine, the theory is you could feel a bit sleepier or more foggy than usual. Realistically that might mean extra drowsiness, dizziness, or feeling slowed down.
I want to be honest about the strength of this one. It is theoretical, based on lab and animal data, and this added sedation has not actually been seen in people. So I would not lose sleep over it, but if you notice unusual grogginess, hold off on driving and check with your pharmacist or doctor before combining them regularly.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Clinical and in vitro research shows that goldenseal can significantly inhibit CYP2D6 enzymes, potentially increasing levels of drugs metabolized by CYP2D6.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with this combo: the codeine part of your medicine is a prodrug. It doesn't do much on its own. Your body has to convert it into morphine using a liver enzyme called CYP2D6, and that's where the pain relief actually comes from.
Goldenseal can block CYP2D6. So instead of building codeine up, it can leave you with less of the active painkiller, meaning weaker relief than you'd expect. The aspirin and caffeine pieces aren't really affected. This is based on some human and lab research, so it's worth taking seriously but it's not an emergency. Occasional culinary amounts are unlikely to matter; concentrated goldenseal supplements are the concern. Check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the wrinkle with this combo product: the codeine part is what really matters. Codeine is a prodrug, meaning your body has to convert it into morphine before you feel any pain relief. That activation step mostly runs through a different enzyme (CYP2D6), while goldenseal mainly slows down CYP3A4, the pathway that breaks codeine down into inactive pieces. So in theory, blocking CYP3A4 could shift more codeine toward the active route, possibly nudging up effects like drowsiness, constipation, nausea, or slowed breathing.
That said, this is based on modest evidence, and the caffeine and aspirin pieces aren't the main concern here. I wouldn't panic, but since opioids carry real risks, please run this past your pharmacist or doctor before pairing the two, especially at concentrated supplement doses.

This combination product contains aspirin, which is a blood thinner that keeps your platelets from clumping together. Goldenseal contains a compound called berberine, and in lab dishes and animal studies it also seemed to make platelets a little less sticky. So the theory is that pairing the two could add up and nudge your bleeding risk a bit higher than aspirin alone.
I want to be honest though: this has not actually been seen in people, so it is more of a what-if than a proven problem. Still, if you notice easy bruising, nosebleeds, bleeding gums, or dark stools, that is worth a call. Check with your pharmacist or doctor before adding goldenseal, especially if you take aspirin regularly.
