Bazedoxifene Acetate, Conjugated Estrogens falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for Duavee (which combines conjugated estrogens with bazedoxifene). Your body relies on an enzyme called CYP3A4 to help break these hormones down. Goldenseal appears to slow that enzyme down, so the medicine may not clear as quickly and levels in your blood could creep up a bit higher than expected.

What might that look like? Possibly more of the usual estrogen-type effects, like breast tenderness, nausea, bloating, or headaches. The evidence here is moderate and comes from limited human studies, so this isn't a guaranteed problem, but it's worth a quick mention. Please don't stop or change anything on your own. Just check with your pharmacist or doctor before pairing goldenseal supplements with Duavee so they can weigh it for you.

P-GLYCOPROTEIN SUBSTRATES

Bazedoxifene Acetate, Conjugated Estrogens also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the situation with Duavee (the combination of bazedoxifene and conjugated estrogens) and goldenseal. The bazedoxifene part is moved around the body by a transporter called P-glycoprotein, and goldenseal may nudge that transporter a little. The catch is that the research pulls in both directions: some lab work suggests goldenseal slightly blocks the pump (which could raise drug levels), while other data suggests it speeds it up. In real people, the effect on a similar drug was small, only about a 14% bump.

So this is mostly a theoretical concern, not a dramatic one. Still, if levels crept up, you might notice more estrogen-type effects like breast tenderness, bloating, or nausea. I wouldn't lose sleep over it, but please run it past your pharmacist or doctor before combining them.

What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
