Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down clomipramine and clear it out. Goldenseal can slow that enzyme down. When the enzyme works more slowly, clomipramine isn't a prodrug, so it simply lingers in your bloodstream longer and levels can creep up.

Higher levels can mean more of the usual tricyclic side effects, like dry mouth, constipation, drowsiness, blurred vision, dizziness on standing, and a faster heartbeat. The evidence here is moderate, not just theoretical, so it's worth taking seriously. Please don't stop or change anything on your own. Just check with your pharmacist or doctor before pairing goldenseal supplements with your clomipramine so they can keep an eye on you.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist: goldenseal contains plant compounds (berberine and hydrastine) that, in test-tube studies, can mildly slow down a liver enzyme called CYP2C9. Since your body uses that enzyme as one of the routes to clear clomipramine, the theory is that goldenseal could let a bit more clomipramine build up in your blood. Clomipramine is a regular medication, not a prodrug, so higher levels would mean stronger effects.

If that happened, you might notice more of clomipramine's usual side effects, like drowsiness, dry mouth, constipation, blurred vision, or a racing heartbeat. The honest part, though, is that this has only been seen in lab dishes, not in real people. I wouldn't panic, but it's worth a quick chat with your pharmacist or doctor before adding goldenseal.

Clomipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

When the cleanup crew works slower, more clomipramine can hang around in your system, and that can nudge up the usual side effects of this tricyclic. The evidence here is moderate, not alarming, and it comes from concentrated goldenseal supplements rather than tiny amounts in food.
