Disopyramide Phosphate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for you. Your body relies on a liver and gut enzyme called CYP3A4 to break down disopyramide and keep its level in a safe range. Goldenseal (mainly its berberine and hydrastine) tends to slow that enzyme down. With the cleanup crew working slower, more disopyramide can hang around in your blood than your dose intends.

That matters a lot here, because disopyramide is a narrow-margin heart rhythm drug. Higher levels can mean a longer QT interval and dangerous rhythm changes, plus more of its anticholinergic effects like dry mouth, blurry vision, constipation, or trouble urinating. The evidence is moderate, not certain, but please don't pair these on your own. Check with your pharmacist or cardiologist first.

What Is Disopyramide Used For?

Brand names: Norpace CR, Norpace

Disopyramide — available as Disopyramide Phosphate, Norpace, and Norpace CR — is indicated for the treatment of documented ventricular arrhythmias, such as sustained ventricular tachycardia (a fast, dangerous heart rhythm originating in the lower chambers), that a physician judges to be life-threatening.

Because of its potential to worsen heart rhythm problems, it is generally not recommended for lesser arrhythmias, and it should not be used to treat ventricular premature contractions (extra heartbeats) that cause no symptoms. Treatment is typically started in a hospital setting.
