Here's the short version: your body relies on a liver enzyme called CYP2D6 to help break down imipramine. Goldenseal can slow that enzyme down, so the medication may clear out of your system more slowly and build up higher than expected. Imipramine isn't a prodrug, so more drug in your blood simply means stronger effects and a greater chance of side effects.

With a tricyclic like this, that can mean things like dry mouth, constipation, blurry vision, dizziness when standing, drowsiness, or a faster heartbeat. The evidence here is real but not airtight, and concentrated goldenseal supplements are far more likely to matter than tiny amounts. Please don't change anything on your own. Check with your pharmacist or doctor first.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the basic idea with goldenseal and your imipramine. Your body uses liver enzymes (one of them called CYP2C9) to break imipramine down and clear it out. In test-tube studies, goldenseal seems to slow that enzyme down a bit. If that were to happen in real life, imipramine could clear more slowly and build up in your system, which can mean stronger side effects like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.

That said, this is only theoretical right now. The slowdown has been seen in the lab, not in people, so we honestly don't know how much it matters day to day. Since imipramine already needs careful dosing, it's worth a quick chat with your pharmacist or doctor before adding goldenseal.

Imipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the simple version: goldenseal can slow down one of your liver's cleanup enzymes (called CYP3A4), and that same enzyme helps break down imipramine. When the enzyme is held back, your body clears the drug a little more slowly, so more of it can build up in your blood.

For imipramine, higher levels can mean stronger side effects like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, and a faster or irregular heartbeat. Imipramine is a tricyclic, and these drugs already need careful dosing, so even a modest bump matters. The evidence here is moderate, not certain, but I'd play it safe. Please check with your pharmacist or doctor before adding goldenseal, and don't change your dose on your own.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
