Irbesartan falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: irbesartan (Avapro) already works to relax your blood vessels and bring your numbers down. Goldenseal contains berberine, which in animal studies and a little human research seems to nudge blood pressure lower on its own. So the worry is mostly additive. Stacking the two could push your pressure down a bit more than expected.

If that happened, you might feel lightheaded, dizzy, or woozy when you stand up. I want to be honest though, this is a theoretical concern based on how berberine behaves, and it hasn't actually been reported with goldenseal itself. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding goldenseal so they can keep an eye on your readings.

Irbesartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the basic idea with goldenseal and irbesartan. Your liver uses an enzyme called CYP2C9 to help clear irbesartan from your body. Goldenseal may gently slow that enzyme down, so in theory a bit more irbesartan could linger in your bloodstream. Since irbesartan lowers blood pressure, higher levels might nudge it lower than expected, which could leave you feeling dizzy, lightheaded, or tired, especially when you stand up quickly.

The honest part: this concern comes only from lab studies, not from real people, so it may not amount to much in practice. Still, it's worth a quick chat with your pharmacist or doctor before adding goldenseal, and keeping an eye on your blood pressure if you do.

What Is Irbesartan Used For?

Brand names: Avapro, Aprovel

Irbesartan tablets — sold as Avapro and under the generic name Irbesartan — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events, including stroke and heart attack. The medication can be used alone or combined with other blood pressure medicines.

Irbesartan tablets are also used to treat diabetic nephropathy (kidney disease caused by diabetes) in adults who have type 2 diabetes, high blood pressure, elevated creatinine levels, and significant protein in the urine. In these patients, it slows the worsening of kidney disease.
