Metformin, Sitagliptin falls under ANTIDIABETES DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypoglycemia when used with antidiabetes drugs.
Goldenseal contains berberine. Clinical research shows that berberine can lower blood glucose levels. However, this effect has not been reported with goldenseal.

Janumet pairs two diabetes medicines: metformin and sitagliptin. Both already work to bring your blood sugar down. Goldenseal contains a compound called berberine, and in studies berberine on its own can also nudge blood sugar lower. Stack that on top of Janumet, and in theory your sugar could dip further than you expect.

I want to be straight with you: this is a theoretical concern based on how berberine behaves, not on real reports of goldenseal causing problems. Still, watch for signs of low blood sugar like shakiness, sweating, dizziness, or feeling suddenly hungry or confused. Neither of these is a prodrug, so this is about an added effect, not changed metabolism. Please check with your pharmacist or doctor before adding goldenseal.

Theoretically, goldenseal might reduce blood levels of metformin.
In vitro research shows that goldenseal extract decreases the bioavailability of metformin, likely by interfering with transport, intestinal permeability, or other processes involved in metformin absorption. It is unclear which, if any, of metformin's transporters are inhibited by goldenseal. Goldenseal does not appear to alter the clearance or half-life of metformin.

If you take Janumet (which combines metformin and sitagliptin) and you're curious about goldenseal, here's the short version. Some early lab work suggests goldenseal may lower how much metformin your body actually absorbs from the gut. If less metformin gets in, your blood sugar control could slip a little, meaning numbers running higher than you'd like. The sitagliptin half isn't really part of this concern.

I'll be honest with you: this is only based on test-tube and lab studies, not real people, so it may not amount to much in everyday life. Still, with a diabetes medicine, it's worth a quick chat with your pharmacist or doctor before adding goldenseal, and keep an eye on your sugar readings if you do.
