Metolazone falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the simple version: metolazone is a water pill that lowers blood pressure by helping your body shed extra salt and fluid. Goldenseal contains a compound called berberine, which in animal studies seems to nudge blood pressure down on its own. Put the two together and, in theory, your blood pressure could drop a bit more than expected.

If that happened, you might feel dizzy, lightheaded, or woozy, especially when standing up quickly. I want to be honest, though: this is a theoretical concern based on how berberine behaves, not something actually proven with goldenseal in people. Still, if you take metolazone and want to try goldenseal, check with your pharmacist or doctor first, and let them know if you notice any dizziness.

What Is Metolazone Used For?

Brand names: Diulo, Zaroxolyn, Mykrox

Metolazone tablets are used to treat edema (excess fluid buildup) linked to congestive heart failure or kidney diseases — including nephrotic syndrome and conditions with reduced kidney function. They are also used to treat high blood pressure (hypertension), either alone or alongside other blood pressure medications.

Metolazone tablets may also be used during pregnancy when edema is caused by a medical condition rather than normal pregnancy changes — but routine use in otherwise healthy pregnant women is not appropriate.
