Perindopril Arginine, Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Prestalia combines two blood pressure medicines: perindopril (an ACE inhibitor) and amlodipine (a calcium channel blocker). Both already work to lower your blood pressure. Goldenseal contains berberine, which in animal studies and some small human research seems to have its own mild blood-pressure-lowering effect. So the worry here is simple: stacking goldenseal on top of Prestalia could, in theory, push your numbers down a little too far.
If that happened, you might feel dizzy, lightheaded, unusually tired, or wobbly when standing up quickly. I want to be honest, though: this is a theoretical concern based mostly on how berberine behaves, not strong proof with goldenseal itself. Still, it's worth a quick chat with your pharmacist or doctor before adding it, and don't change your Prestalia on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Perindopril Arginine, Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Your blood pressure pill, Prestalia, combines two medicines. The amlodipine half is the one to watch here, because your body clears it using a liver enzyme called CYP3A4. Goldenseal tends to slow that enzyme down, so amlodipine can clear more slowly and build up a bit in your system.
If amlodipine levels creep up, you may notice more of its usual effects, like dizziness, flushing, swollen ankles, or feeling lightheaded when you stand. The perindopril side is less affected, but lower blood pressure overall is still possible. The evidence here is moderate, not alarming, and short-term or occasional use is different from daily concentrated capsules. Before starting goldenseal, please check with your pharmacist or doctor so they can keep an eye on your readings.
