Sacubitril, Valsartan falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: Entresto (sacubitril/valsartan) already lowers your blood pressure as part of helping your heart. Goldenseal contains a compound called berberine that, at least in animal studies and in one human study pairing berberine with a different blood pressure pill, may nudge blood pressure down a bit further. Put them together and the worry is simply too much of a good thing.

If your pressure drops too low, you might feel dizzy, lightheaded, faint, or unusually tired, especially when standing up quickly. I'll be honest with you: this concern is mostly theoretical, and it hasn't actually been reported with goldenseal itself. Still, since Entresto is a heart medication, please check with your pharmacist or doctor before adding goldenseal so they can keep an eye on your readings.

Sacubitril, Valsartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist with Entresto (sacubitril/valsartan) and goldenseal. Part of this medication, the valsartan piece, is partly handled by a liver enzyme called CYP2C9. Goldenseal may gently slow that same enzyme down. If that happens, slightly more of the drug could linger in your blood, which in theory might nudge its effects up a bit.

For Entresto, stronger effects could mean things like lightheadedness, dizziness, low blood pressure, or higher potassium. But here's the honest part: this concern comes only from lab studies, not real people, so it's largely theoretical. It may not amount to much in practice. Still, since Entresto is a heart failure medicine, I'd check with your pharmacist or doctor before adding goldenseal.

What Is Valsartan and Sacubitril Used For?

Brand name: Entresto

Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).

It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
