Treprostinil (Oral) falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the thing to know about pairing goldenseal with oral treprostinil (Orenitram). Treprostinil already works by widening blood vessels to ease the strain in your lungs, and a common side effect of that is lower blood pressure. Goldenseal contains berberine, which in lab and animal studies seems to have its own mild blood-pressure-lowering effect. So in theory, stacking the two could nudge your blood pressure down a little further than expected.

I want to be honest about how soft this evidence is. This is a theoretical concern based on pharmacology, not something proven with goldenseal in real patients. Still, if it added up, you might feel more lightheaded, dizzy, flushed, or headachey, especially when standing up. Please don't change anything on your own. Just check with your pharmacist or doctor before adding goldenseal.

Here's the situation with combining goldenseal and oral treprostinil (Remodulin). Treprostinil works by relaxing and widening blood vessels in the lungs, and one of its known effects is that it can make your blood a little less likely to clot, so it already carries some bleeding risk on its own. Goldenseal contains berberine, and in lab dishes and animal studies berberine seems to slow platelets from sticking together. Stack those together and, in theory, you could see a slightly higher chance of bruising or bleeding.

I want to be honest though: this effect has not actually been reported in people, so it's a theoretical concern based on how things work, not proven harm. Still, since both can nudge bleeding the same direction, please check with your pharmacist or doctor before adding goldenseal rather than starting it on your own.

About Treprostinil

What Is Treprostinil Used For?

Brand names: Remodulin, Orenitram

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
