Why this applies to Triamcinolone Acetonide

Triamcinolone Acetonide falls under P-GLYCOPROTEIN SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the honest picture: triamcinolone acetonide is moved around the body in part by a transporter called P-glycoprotein, and goldenseal can nudge that transporter. The catch is that the research goes both ways. In lab studies berberine (goldenseal's main compound) slightly blocks this transporter, while other data suggest it speeds it up. In real people, goldenseal only bumped one P-gp drug (digoxin) by about 14%, which is small.

For your situation, this matters less than it might sound. The Zilretta (knee injection) and Xipere (eye injection) forms act mostly where they're placed, so a mild P-gp effect is unlikely to cause much. Still, if you ever notice extra steroid effects like fluid retention, mood changes, or higher blood sugar, loop in your pharmacist or doctor before combining them.
