Mometasone, formoterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Your Zenhale inhaler combines two medicines, and one of them, mometasone (an inhaled steroid), is partly cleared by a liver enzyme called CYP3A4. The berberine in goldthread might slow that enzyme down. In theory, if mometasone clears more slowly, a bit more could end up circulating in your body.

I want to be honest about how shaky this is. This interaction is only a lab-based, theoretical concern, and it has not actually been reported in people. Since you inhale only a small dose, a real problem is unlikely. Still, with steroids, watch for things like oral thrush, hoarseness, or signs of higher steroid exposure over time. Before adding a concentrated goldthread or berberine supplement, just check with your pharmacist or doctor.
