Perampanel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for you: your seizure medicine perampanel (Fycompa) is cleared from your body mainly by a liver enzyme called CYP3A4. Berberine, the active compound in goldthread, may gently slow that same enzyme down. If clearance slows, more perampanel can build up than expected, which could mean stronger effects and side effects like dizziness, drowsiness, trouble with balance, or mood and irritability changes.

I want to keep this in perspective, though. This is only a theoretical concern based on lab thinking, and it hasn't actually been reported in people. Still, since perampanel is taken steadily and side effects can sneak up, I'd run goldthread by your pharmacist or neurologist before adding it rather than starting it on your own.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
