Vemurafenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for vemurafenib (Zelboraf): your body partly clears this cancer medicine using a liver enzyme called CYP3A4. Goldthread contains berberine, which lab research hints may slow that same enzyme down. If that happened, vemurafenib could clear more slowly and build up in your system, which can mean more of its known side effects, like skin rashes, sun sensitivity, joint pain, fatigue, and sometimes changes in heart rhythm or liver tests.

I want to be honest about the evidence here. This is theoretical, based on how these compounds behave in the lab, and it has not actually been reported in people. Still, because vemurafenib is a serious cancer drug with real monitoring needs, please loop in your oncologist or pharmacist before adding goldthread rather than guessing.

About Vemurafenib

What Is Vemurafenib Used For?

Brand name: Zelboraf

Zelboraf (vemurafenib) is used to treat adults with unresectable or metastatic melanoma — meaning melanoma that cannot be surgically removed or has spread — when the tumor tests positive for a specific genetic change called the BRAF V600E mutation. An FDA-approved test must confirm this mutation before starting treatment.

Zelboraf is also approved for patients with Erdheim-Chester Disease (ECD), a rare condition where abnormal immune cells build up in organs, when that disease carries a BRAF V600 mutation. Zelboraf is not for patients whose melanoma has normal (wild-type) BRAF, as it may actually promote tumor growth in that setting.
