Hawthorn and Aspirin, Hydrocodone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Aspirin, Hydrocodone

Aspirin, Hydrocodone falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hawthorn interacts with.

Theoretically, hawthorn may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that hawthorn can inhibit platelet aggregation. However, its effect in humans is unclear. One observational study shows that patients taking hawthorn shortly before undergoing coronary artery bypass graft (CABG) surgery or valve replacement surgery have a 10% incidence of postoperative bleeding, compared with 1% in those who never consumed hawthorn extract. However, clinical research shows that taking a specific preparation of dried hawthorn leaves and flowers (Crataesor, Soria Natural Lab) 800 mg three times daily for 15 days does not affect platelet aggregation or levels of thromboxane B2, the metabolite of thromboxane A2, in healthy humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing with this combo. Your medication pairs aspirin (a blood thinner that keeps platelets from clumping) with hydrocodone (an opioid pain reliever). The concern is really about the aspirin half. Lab and animal studies hint that hawthorn might also make platelets a little less sticky, so stacking it on top of aspirin could, in theory, nudge your bleeding risk up a bit.

I want to be honest though: this is a theoretical worry. The human evidence is thin and even a little contradictory, with one healthy-volunteer study showing no real effect on platelets. Still, if you notice easy bruising, nosebleeds, or bleeding gums, or you have surgery coming up, mention the hawthorn to your pharmacist or doctor first. Don't start or stop anything on your own.

From our pharmacist’s overview on the Aspirin, Hydrocodone page

“Combining supplements with aspirin and hydrocodone (Lortab ASA, Alor 5 500) deserves real vigilance, because this medication carries two separate kinds of risk. Aspirin thins the blood, and by far the largest share of these interactions can add to that effect and raise bleeding risk.”

About Hawthorn

What is Hawthorn?

Hawthorn
  • Heart failure symptoms
  • High blood pressure
  • Chest pain (angina)
  • Irregular heartbeat
  • Digestive upset
  • Anxiety

Hawthorn is a plant traditionally used for heart-related complaints, and some studies suggest it may modestly help symptoms of mild heart failure when added to standard care. However, the evidence is mixed and it should never replace prescribed heart medicines. Always talk to your pharmacist or doctor before using hawthorn, especially if you have a heart condition.
