Hawthorn and Ibuprofen, Acetaminophen

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Ibuprofen, Acetaminophen

Ibuprofen, Acetaminophen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hawthorn interacts with.

Theoretically, hawthorn may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that hawthorn can inhibit platelet aggregation. However, its effect in humans is unclear. One observational study shows that patients taking hawthorn shortly before undergoing coronary artery bypass graft (CABG) surgery or valve replacement surgery have a 10% incidence of postoperative bleeding, compared with 1% in those who never consumed hawthorn extract. However, clinical research shows that taking a specific preparation of dried hawthorn leaves and flowers (Crataesor, Soria Natural Lab) 800 mg three times daily for 15 days does not affect platelet aggregation or levels of thromboxane B2, the metabolite of thromboxane A2, in healthy humans.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical picture with Advil Dual Action, which combines ibuprofen and acetaminophen. The ibuprofen part is the one that matters here. Ibuprofen is an NSAID that already nudges your blood toward bleeding more easily by reducing how well platelets stick together, and it can irritate the stomach lining too. Lab and animal studies hint that hawthorn might also slow platelets a little, so in theory stacking the two could add up to a bit more bleeding or bruising risk, like nosebleeds, easy bruising, or stomach upset.

I want to be honest, though: this is a theoretical concern based mostly on test-tube and animal work, and human studies haven't clearly shown a problem. It's worth mentioning to your pharmacist or doctor, especially before any surgery, but it's not a reason to panic.

From our pharmacist’s overview on the Ibuprofen, Acetaminophen page

“Advil Dual Action pairs ibuprofen, an NSAID with built-in stomach-bleeding risk, and acetaminophen, which the liver processes, so this interaction list deserves a careful read, not a skim. Most of the entries can add to bleeding risk, while smaller clusters put extra strain on the liver or kidneys.”

About Hawthorn

What is Hawthorn?

Hawthorn
  • Heart failure symptoms
  • High blood pressure
  • Chest pain (angina)
  • Irregular heartbeat
  • Digestive upset
  • Anxiety

Hawthorn is a plant traditionally used for heart-related complaints, and some studies suggest it may modestly help symptoms of mild heart failure when added to standard care. However, the evidence is mixed and it should never replace prescribed heart medicines. Always talk to your pharmacist or doctor before using hawthorn, especially if you have a heart condition.
