Rifapentine falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA) which are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the gist of this pairing. Rifapentine is a strong CYP3A4 inducer, meaning it revs up that liver enzyme system. Hemp agrimony can contain pyrrolizidine alkaloids, plant compounds that the liver processes through that same CYP3A4 pathway. The worry is that by speeding up that enzyme, rifapentine could push more of those alkaloids into their toxic, liver-damaging forms. In other words, the concern here is added strain on your liver, not a change in how well your TB medicine works.

I'll be honest, this is based on lab and theory, not real patient studies, so the risk is uncertain. But since hemp agrimony itself can be hard on the liver, and your TB regimen already gets watched closely for liver effects, I'd skip the hemp agrimony and check with your doctor or pharmacist first.

Hemp agrimony is a traditional European herb that has been used for digestion, colds, and as a general tonic, but there is very little reliable human research to support these uses. More importantly, the plant can contain pyrrolizidine alkaloids that may harm the liver, so most experts do not recommend taking it internally. About Rifapentine

What Is Rifapentine Used For?

Brand name: Priftin

Rifapentine (brand name Priftin) is used to treat two forms of tuberculosis (TB) caused by Mycobacterium tuberculosis. For active pulmonary TB — an active infection in the lungs — it is approved for patients 12 years and older, always used alongside other TB medicines, never alone. For latent TB infection (LTBI) — where the bacteria are in the body but not causing active illness — it is approved for patients 2 years and older who are at high risk of the infection becoming active, always combined with isoniazid.

Priftin is not appropriate for every TB situation. It should not be used alone at any stage of active TB treatment, and it is not recommended as a once-weekly continuation regimen combined with isoniazid in people living with HIV who have active pulmonary TB, due to a higher risk of treatment failure or relapse.
