Selpercatinib falls under HEPATOTOXIC DRUGS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA). Theoretically, using hemp agrimony with hepatotoxic drugs might have additive adverse effects on the liver. Some drugs that can adversely affect the liver include acetaminophen (Tylenol and others), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), fluconazole (Diflucan), itraconazole (Sporanox), erythromycin (Erythrocin, Ilosone, others), phenytoin (Dilantin), lovastatin (Mevacor), pravastatin (Pravachol), simvastatin (Zocor), and many others.

Here's the concern with combining these two. Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds that may be hard on the liver. Your cancer medicine, selpercatinib (Retevmo), can also stress the liver on its own. In fact, your care team likely checks your liver blood tests (like ALT and AST) regularly while you're on it. Stacking two liver stressors together could, in theory, add up and raise those numbers more.

I'll be honest with you: this is a theoretical caution based on how each acts, not something proven in studies. Still, with a serious cancer drug like this, I'd play it safe. Please don't start or stop anything on your own, just loop in your oncologist or pharmacist first.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
