Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA) which are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the part folks often get backwards with this pairing. The concern isn't that hemp agrimony changes your zanubrutinib (Brukinsa) levels. It's actually the other way around. Hemp agrimony contains pyrrolizidine alkaloids, which are natural compounds that can be hard on the liver. Zanubrutinib is a mild inducer of the CYP3A4 enzyme, meaning it can speed up how the body processes those alkaloids. In theory, that faster processing could create more of the toxic, liver-damaging byproducts.

I want to be honest about the evidence here, though. This is based only on lab and animal data, so it's a theoretical worry, not something proven in people. Even so, hemp agrimony already carries real liver-safety concerns, and zanubrutinib itself is monitored for liver effects. I'd skip the hemp agrimony and check with your oncologist or pharmacist first.

What Is Zanubrutinib Used For?

Brukinsa (zanubrutinib) is used to treat adults with several types of blood cancers. These include chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), Waldenström's macroglobulinemia (WM), mantle cell lymphoma (MCL) after at least one prior treatment, relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 treatment, and relapsed or refractory follicular lymphoma (FL) — the last in combination with another medicine called obinutuzumab, after two or more prior treatments.

Some of these uses (MCL, MZL, and FL) have received accelerated FDA approval based on response rates, meaning they may be subject to further confirmation of long-term benefit in ongoing trials.
