Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Darunavir, Cobicistat
Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Prezcobix (darunavir plus cobicistat) and hibiscus. Both of these medicines lean heavily on a liver enzyme called CYP3A4. In fact, cobicistat is added on purpose to slow that enzyme down so darunavir stays at higher, more effective levels. In lab dish studies, hibiscus extract also appears to block CYP3A4 a little. In theory, that could nudge darunavir levels up a bit, which might mean more of its usual side effects like nausea, diarrhea, headache, or rash.
The honest truth is this is only a theoretical concern. It hasn't been seen in real people, and an occasional cup of tart hibiscus tea is unlikely to cause trouble. Concentrated hibiscus supplements are the bigger question mark. Since staying on track with HIV therapy matters, just run any regular hibiscus use by your pharmacist or doctor first.

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure.
