Hibiscus Sabdariffa + Fexofenadine

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Fexofenadine
Fexofenadine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest version: this concern comes from lab dish studies only, where hibiscus extract slowed an enzyme called CYP3A4. The thing is, fexofenadine (Allegra) barely depends on that enzyme. It's mostly absorbed and removed unchanged by your body, not broken down much at all, so this particular pathway isn't a big deal for it.
If anything ever shifted, you might notice slightly more of fexofenadine's usual mild effects, like a touch of headache or drowsiness, but that's unlikely. This has never been reported in real people. Enjoy your hibiscus tea without much worry. If you take concentrated hibiscus capsules regularly, just mention it to your pharmacist so they can double-check your whole list.

From our pharmacist’s overview on the Fexofenadine page
“An over-the-counter allergy pill sounds like the last place serious interactions would hide, but fexofenadine (Allegra) has 154 supplements and foods on file, and the five rated major deserve real attention before you combine it with much of anything else. Two of those five sit in most refrigerators.”

About Hibiscus Sabdariffa
What is Hibiscus Sabdariffa?
- High blood pressure
- Heart and cholesterol health
- Digestive upset
- Antioxidant support
- Fever and general wellness
Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. Evidence for other uses is limited, and while hibiscus tea is generally well tolerated in food amounts, concentrated supplements should be used cautiously and avoided in pregnancy. Talk to your pharmacist or doctor before using it for a medical condition.
About Fexofenadine
What Is Fexofenadine Used For?
Brand name: Allegra
Fexofenadine temporarily relieves symptoms of hay fever (seasonal allergic rhinitis) and other upper respiratory allergies, including runny nose, sneezing, itchy or watery eyes, and itching of the nose or throat.
Several products — including Allegra Hives 24hr tablets — are specifically used to reduce hives (urticaria) and relieve the itching that comes with them. Keep in mind that fexofenadine will not prevent hives or stop an allergic skin reaction from happening in the first place.
