Guaifenesin, Hydrocodone Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the interesting wrinkle with this combo. The hydrocodone part of your medicine relies on an enzyme called CYP2D6 to turn into a stronger painkiller called hydromorphone. Lab studies (just test-tube work, not in real people) suggest hibiscus may slow that enzyme down. So in theory, hibiscus could mean less of that conversion, which might make the cough and pain relief feel a bit weaker rather than stronger.
Honestly, this is very theoretical and rated as minor, so a cup or two of hibiscus tea is unlikely to cause a noticeable problem. Concentrated hibiscus supplements are the bigger question mark. If you're relying on this medicine and want to use hibiscus regularly, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cough product pairs guaifenesin (which loosens mucus) with hydrocodone, an opioid that quiets coughing and eases pain. The hydrocodone part is partly handled by a liver enzyme called CYP3A4. Lab studies suggest concentrated hibiscus extract may slow that enzyme down a bit, which could let a little more hydrocodone linger in your system.
If that happened, you might feel more of the usual opioid effects, such as extra drowsiness, dizziness, constipation, or slowed breathing. That said, this is only a theoretical concern from test-tube work, not something seen in real people, and an occasional cup of tart hibiscus tea is unlikely to matter. Concentrated supplements are where I'd be more cautious. If you use hibiscus regularly, just run it by your pharmacist or doctor.
