Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for Erleada (apalutamide). Your body uses a liver enzyme called CYP3A4 to help process this drug, and lab and animal studies hint that hops might gently speed that enzyme up. If that happened to a meaningful degree, apalutamide could be cleared a little faster, which in theory might lessen how well it controls your prostate cancer.

The good news: this is very weak, mostly test-tube and animal evidence. In a human study, a hops extract changed a similar CYP3A4 drug by under 8 percent, which is too small to matter. So I wouldn't lose sleep over it. Still, apalutamide is serious cancer therapy, so please run any hops supplement past your oncology team or pharmacist before starting.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
