Why this applies to Perindopril Arginine, Amlodipine

Perindopril Arginine, Amlodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist with Prestalia, which combines amlodipine and perindopril. The amlodipine half is cleared by a liver enzyme called CYP3A4, and hops has been shown in lab and animal work to mildly speed that enzyme up. In theory, that could clear amlodipine a touch faster and slightly soften its blood-pressure-lowering effect. (Perindopril is actually a prodrug, but it's activated by a different pathway, so hops isn't the concern there.)

In real life this is very weak. A human study found only about an 8% change in a similar drug, which isn't enough to matter for most people. I wouldn't lose sleep over an occasional hops tea or sleep blend. If you use concentrated hops daily, just mention it to your pharmacist and keep an eye on your home blood pressure readings.
