Aspirin, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The caffeine part of Anacin is the piece to think about here. Your body clears caffeine mainly through a liver enzyme called CYP1A2. In test-tube studies, horny goat weed appears to slow that same enzyme down. If that were to happen in real life, caffeine could hang around longer and build up, which might leave you feeling jittery, wired, or sped up, with a faster heartbeat, trouble sleeping, or an upset stomach. The aspirin portion isn't really affected this way.
Here's the reassuring part: this is only seen in lab work, not in actual people, so it's theoretical and on the weaker side of the evidence. Still, if you take this combo and notice more caffeine-type side effects, it's worth a quick chat with your pharmacist or doctor.

In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the thing to keep in mind with Anacin: the aspirin part already thins your blood a bit by making platelets less sticky, which is exactly why it helps prevent clots. In lab dishes and animal studies, horny goat weed seems to do something similar, lowering how well platelets clump together. Stack the two and, in theory, you could nudge your bleeding risk a little higher.
I want to be honest about the evidence though: this has only been seen in test tubes and animals, never reported in people, so it's more of a caution than a proven problem. Still, watch for easy bruising, nosebleeds, or a cut that won't stop. If you take aspirin daily, just run the horny goat weed past your pharmacist or doctor first.

Here's the gist with Anacin (aspirin plus caffeine) and horny goat weed. In test tube studies, the herb appears to slow down a liver enzyme called CYP3A4. If that happened in real people, drugs cleared by that enzyme could build up a bit and hit harder. With this combo, the caffeine portion is the piece most handled by CYP3A4, so in theory you might feel more caffeine-like effects, think jitteriness, a racing or pounding heart, or trouble sleeping.
That said, I want to be honest: this is only lab and animal data, and it has not been shown in humans. So it's more of a theoretical heads-up than a proven problem. If you take Anacin regularly, just run it by your pharmacist or doctor before adding the herb.
