Eplerenone falls under ANTIHYPERTENSIVE DRUGS — the group Horny Goat Weed interacts with.
Laboratory research suggests that horny goat weed might have hypotensive effects. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: eplerenone already works to lower your blood pressure, and lab research hints that horny goat weed may have a mild blood-pressure-lowering effect of its own. If both nudge your pressure in the same direction, the worry is that it could dip a little too low. That might leave you feeling dizzy, lightheaded, or unsteady when you stand up, and in some cases unusually tired.
I'll be honest with you though, this is based on test-tube and animal studies, not real reports in people, so it's more of a theoretical caution than a proven problem. Eplerenone isn't a prodrug here, so the herb isn't changing how it's activated. Still, since safety data on this herb is thin, please run it by your pharmacist or doctor before combining them.

Eplerenone treats two conditions. First, it is used to lower high blood pressure (hypertension) in adults. Lowering blood pressure reduces the risk of stroke and heart attack. It can be used alone or alongside other blood pressure medicines.
Second, eplerenone is used to improve survival in stable adults who have heart failure with reduced ejection fraction (HFrEF) — meaning the heart pumps less blood than normal — following an acute myocardial infarction (heart attack).
