Eplerenone falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the thing to keep in mind if you're using adrafinil while taking eplerenone (Inspra) for your blood pressure. Your body turns adrafinil into modafinil, a wakefulness-promoting stimulant. Modafinil has been shown to nudge blood pressure upward in some people. Since eplerenone is working to lower your blood pressure, adrafinil could push in the opposite direction and partly blunt how well your medication does its job.

I'll be straight with you: this concern is theoretical, based on how these drugs work rather than strong studies, so it may not happen at all. Still, if you choose to use adrafinil, it's worth checking your blood pressure at home and letting your doctor or pharmacist know, so they can keep an eye on your numbers.

What Is Eplerenone Used For?

Brand name: Inspra

Eplerenone treats two conditions. First, it is used to lower high blood pressure (hypertension) in adults. Lowering blood pressure reduces the risk of stroke and heart attack. It can be used alone or alongside other blood pressure medicines.

Second, eplerenone is used to improve survival in stable adults who have heart failure with reduced ejection fraction (HFrEF) — meaning the heart pumps less blood than normal — following an acute myocardial infarction (heart attack).
