Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist with tadalafil (Cialis). Your liver uses an enzyme called CYP3A4 to clear it from your body. In test-tube studies, horny goat weed appears to slow that enzyme down. If that happened in people, your body might break down tadalafil more slowly, so more of it could hang around. That could mean stronger effects and a bigger chance of side effects like headache, flushing, a stuffy nose, back pain, or feeling lightheaded from a drop in blood pressure.

I want to be honest about the evidence though: this is only seen in the lab, not in actual patients, so it's theoretical at this point. On top of that, horny goat weed is sometimes promoted for the same ED purpose as tadalafil, so people occasionally stack them. I'd run that combo past your pharmacist or doctor first rather than guessing.

About Tadalafil

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
