Here is the interaction we found between these two, with the evidence behind it.
ANTICOAGULANT/ANTIPLATELET DRUGS
Why this applies to Abrocitinib
Abrocitinib falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Ilex Guayusa interacts with.
Ilex guayusa contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, caffeine in Ilex guayusa might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture with guayusa tea and Abrocitinib (Cibinqo). The concern on file is really about the caffeine in guayusa, which may have a mild blood-thinning (antiplatelet) effect. Abrocitinib itself can already lower your platelet counts as a known side effect, so the idea is that the two together might slightly raise bleeding risk.
I want to keep this in perspective. This is a theoretical interaction based on how caffeine behaves, not something actually reported in people, and it's rated unlikely. A normal cup or two of guayusa is unlikely to be a problem for most folks. Still, if you notice easy bruising, nosebleeds, or bleeding gums, or you drink large amounts daily, check in with your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Abrocitinib page
“Bleeding is where I'd focus first with abrocitinib (Cibinqo), an eczema pill that can itself lower platelet counts. Of the 290 supplements and herbs with an interaction on file, 202 have blood-thinning or antiplatelet activity, and two of the three major-rated entries, danshen and piracetam, sit in that group.”

About Ilex Guayusa
What is Ilex Guayusa?
- Energy and alertness
- Mental focus
- Antioxidant support
- Appetite control
- Athletic and physical performance
Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. Because it is a stimulant, use it cautiously and check with your pharmacist or doctor if you have heart concerns or are pregnant.
